DR. RICHARD KOFFLER Board Certified Physiatrist 

Upon completing his residencies, Dr. Koffler came to the realization that most, if not all, disease states are a consequence of what has been brewing underneath, are multifactorial and outside of trauma most of what ails us start in the emotional state (i.e. how you handle and perceive life). You are what you think and you are what you eat. 

Therefore, Dr. Koffler began to look at the importance of diet, exercise and mental status on overall well-being. It is his philosophy that to be a caring physician, you must see the patient as an individual, empathize with them and treat them as a whole. Dr. Koffler tries to integrate Western as well as alternative/complementary approaches to whole health through the appropriate balance of nutrition, exercise, and acupuncture for the purpose of addressing issues such as stress, fatigue, anti-aging, musculoskeletal and nerve related problems. He treats patients the way he would like to be treated. What ails one person may not have the same root cause as someone else although his or her symptoms may appear similar. That is the primary reason Dr. Koffler spends the time getting to know the concerns and issues a patient may have. The current allopathic medical model tends to pigeonhole symptoms into a diagnosis that is convenient and then forces it to fit into the insurance model of one-diagnosis fits all with a generic pill or two to fix it. Contrary to that model, Dr. Koffler will listen to the patient, ask pertinent questions and educate them on their condition. He then will create a health plan with the patient that educates and empowers. When necessary, he will work with other quality, alternative as well as conventional practitioners that he can trust to make sure the patient receives what they need in order to heal. Dr. Koffler wants patients to see him as a resource in empowering them to obtain and maintain optimal health. This is why he is continually furthering his education in Nutrition, Age Management Medicine, Functional Medicine, and Mind-Body Wellness.  MICHAEL MASE R.N. Registered Nurse, Injector, Clinical Educator



 Michael has a degree in mechanical engineering from Roger Williams University, which he received in 2008. Michael worked in the field for two years and then decided to pursue a career in nursing. Michael attended Naugatuck Valley Community College and passed his NCLEX examination in July of 2013. From there Michael spent two years working night shifts supervising a nursing home and working on the oncology floor at Danbury hospital. In January of 2014, Michael attended the aesthetics skin institute and became certified in Botox and dermal fillers. Shortly later that year Michael began working as a nurse injector at Rejuva Skin and Laser Spa. Then in January 2016, Michael began work at Vivacity Life center in Stamford, CT. In February 2017, Michael began work as a nurse injector at Revive Beauty and Wellness in Brookfield, Ct. That very same year in October Michael was approached by the MERZ corporation to work as a clinical educator and conduct training on the FDA approved uses of Xeomin, Radiesse, and Belotero. Michael traveled all across the east coast conducting training to at least 100 different offices over a year and a half time span. In August of 2018, Michael accepted a full-time position with Merz on an elite twelve member team called the Merz Aesthetic Consultants. Since then Michael has parted ways with Merz to focus strictly on the practice of cosmetic nursing. Currently, Michael is aspiring to complete his Nurse practitioners degree in adult health.

JESSICA R. DACOSTA MSN, AGNP-C

Jessica holds both a Master's and Bachelor's degree in nursing from Western Connecticut State University and is currently a board-certified Adult Gerontology Nurse Practitioner (AGNP, APRN) in Connecticut. With a background as a critical care RN at Norwalk Hospital, Jessica has extensive experience in post-critical care and intensive care units, caring for a diverse range of patients. Her commitment to professional development led her to Aesthetic Medical Educators Training (AMET), where she obtained her first certificate. Specializing in aesthetic medicine, Jessica has completed various certifications in neuromodulators (e.g., Botox) and dermal fillers, gaining not only theoretical knowledge but also practical expertise. Her hands-on training ensures efficient and natural results for her patients. In addition to mastering Botox and dermal filler procedures, Jessica is skilled in traditional micro-needling, micro-needling with or without platelet-rich plasma, and micro-needling combined with radio-frequency therapy using Morpheus8. She is certified to perform Ultherapy and PDO threading procedures, showcasing her versatility in aesthetic treatments. Driven by a passion for both medicine and beauty, Jessica seamlessly combines the two as an aesthetic injector. Committed to staying abreast of the latest techniques, she adapts to her patients' evolving needs. Jessica's ultimate goal is to create a comfortable experience for her clients, leaving them feeling their very best in their own skin.

SOPHIA RICKEVICIUS, APRN, Aesthetic Injector

Sophia started working at Revive back in April of 2020. She has been a Family Nurse Practitioner since 2017, and worked as a registered nurse prior to that for 8 years, the majority of which spent working in the emergency department. As a nurse practitioner I have worked in geriatrics and urgent care. I received my bachelors of biology and nursing in 2009 at St. Joseph’s College and then my family nurse practitioner in 2017 at University of St. Joseph’s. Sophia has been working alongside Revive’s injectors learning the best techniques and is swiftly growing a following clientele at Revive. She excels in neurotoxin and filler administration, Morpheus 8, Ultherapy, PRP Microneedling and Evoke.

LIA MAZZADRA, RN, BSN

 Lia Mazzadra, RN ,BSN comes to Revive as a certified aesthetic nurse and certified nurse injector along with an expertise in laser and laser technology. Lia strives to help her clients to gain their confidence by providing them with results driven treatments towards their goals. With each one of her clients she wants them to leave feeling better than when they first walked in. Lia is the perfect addition to Revive Beauty and Wellness providing laser treatments, along with injectables and multiple different anti-aging aesthetic procedures.
